On the front of this three color, single sided Phoenix Program ID card, there is room for a photo, rank as well as a card number. With nothing printed on the reverse side of this card, the front also states the above named: served with honor and distinction as a member of the PHOENIX PROGRAM during the time period listed. It also states that: DUE TO THE CLASSIFIED NATURE OF THIS PROGRAM, THE BEARER IS PROHIBITED FROM MAKING ANY STATEMENT CONCERNING THE NATURE OF HIS MISSION.
