Our ID-A-CHILD identification card provides the most critical information should you ever have the need. The front side has fields for child’s name, issue date, D.O.B., height, hair, eyes, weight, parents/guardians name, city/state/zip code, telephone number and picture. The backside has a box for the child’s right thumbprint. There are also instructions on what to do should your child ever become lost. We hope there will never be a need for you to need this information, but it is important to have the information available in case of an emergency. This ID-A-Child Identification card measures a standard size of 2 ¼-inches X 3 ¾-inches, which fits perfectly in your wallet or organizer.
